How Oracle's $37 Billion Failure Created SENTINEL
Two documents in this Command Center tell the complete story. The first — Oracle Federal Contract Failure Record — covers 20 years across VA, DoD, GSA, and Oregon: $13.84 billion obligated for the VA EHR alone, 6 of 171 facilities live after 8 years, veterans fatally harmed, 11,000 clinical orders routed to an unknown queue, FBI investigation in Oregon, $280 million DoD HR system terminated by DOGE, 9 of 10 GAO priority recommendations unimplemented. The second — VA IT Infrastructure and Oracle Contract Analysis FY2026 — shows what happened next: Oracle's contract was doubled to $2.2 billion while DOGE terminated the 6 veteran-owned small businesses providing support, including the HIPAA compliance vendor and the DoD interoperability contractor.
That pattern — a system that cannot see the veteran's record, cannot apply the correct law, and cannot be held accountable when it fails — is structural, not incidental. VistA runs in 130 separate implementations across 171 facilities. They do not talk to each other. A rating change in VBA does not reach VHA in real time. A denial gets written citing law that never applied to that veteran — and there is no architectural mechanism to catch it. That gap is what SENTINEL was built to close. The architecture Oracle was paid $37 billion to create and did not deliver — SENTINEL delivers it from the ground up, built right, built to a statutory standard, and built so no future Oracle can happen again.

The Fragmentation Problem
Oracle/Cerner's EHR replaced nothing systemically. VistA still runs at 94% of VA facilities. The two systems do not synchronize reliably — a veteran treated at an Oracle site whose medications are checked at a VistA site requires manual pharmacist safety verification because the automated process cannot be trusted. That is not a software bug. That is an architectural failure to unify the data. SENTINEL's response: mandatory 15-minute synchronization ceiling between VBA and VHA databases, by statute, with automatic breach consequences if violated.

The Denial Pattern
VA OIG audits and GAO reports document a consistent pattern: veterans denied care through citation of policies that do not apply to them. Pediatric policies applied to adult prosthetics orders. MST care denied for absence of documentation the law never required. These are systemic — caused by the correct law not being on the screen when the decision is made. Oracle did not solve this. SENTINEL's response: Program Coverage Schedule displays only law applicable to this veteran, this condition, this transaction. Law not applicable cannot be cited because it was not on the screen.

The Accountability Vacuum
Oracle received $4.4 billion through FY2025 and refunded $325,000. The Inspector General issued 70+ recommendations — most unimplemented. 58% of VA staff using Oracle's system believe it increased patient safety risk. There is no mechanism to automatically catch a bad denial, record who made it, and escalate without human intervention. SENTINEL's response: four-layer VETSAFE-Grade verification, immutable audit trail by name, Inaction-as-Decision — consequences trigger from the clock, not from a human deciding to impose them.

Neither Oracle's EHR nor VistA has a mechanism to automatically notify a veteran's family of benefits owed at time of death. The family must know to ask. They must know what to ask for.
